{"id":6101,"date":"2023-09-29T16:31:54","date_gmt":"2023-09-29T20:31:54","guid":{"rendered":"https:\/\/aux.charlotte.edu\/?page_id=6101"},"modified":"2026-01-22T14:18:24","modified_gmt":"2026-01-22T19:18:24","slug":"minor-passports","status":"publish","type":"page","link":"https:\/\/cws.charlotte.edu\/backup-aux\/mail-package-services\/passport-services\/minor-passports\/","title":{"rendered":"Minor Passports"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\">Applicants and their parent(s) must be present to apply for a U.S. passport.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Applicant Information<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Both parents\/legal guardians must be present and show valid photo identification<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>If a parent cannot be present, then a <a href=\"https:\/\/eforms.state.gov\/Forms\/ds3053.pdf\" target=\"_blank\" rel=\"noreferrer noopener\">DS-3053 Parental Consent Form<\/a> (notarized) from the absent parent is required with a photocopy of front and back of ID is required. When all parents\/guardians are unable to be present at the time of execution, they can complete the <a href=\"https:\/\/eforms.state.gov\/Forms\/ds3053.pdf\">DS-3053<\/a>. Section 4B of the consent form should list the name of the person appearing with the minor.<\/li>\n\n\n\n<li>A consent form must be submitted from both parents, with front and back copy of their ID.<\/li>\n\n\n\n<li>Proof of U.S. Citizenship: Original or Certified copy of Birth Certificate with a raised embossed seal<\/li>\n\n\n\n<li>Consular Report of Birth Abroad <\/li>\n\n\n\n<li>Certificate of Naturalization with identifiable photo attached or a previous passport<\/li>\n\n\n\n<li>Birth Certificate is required for minor\u2019s renewing their 5-year passport. <strong>Old passports must be submitted with new application<\/strong>. A minor using a current or previous passport as proof of citizenship, must also submit a photocopy of their birth certificate to establish the relationship of the person signing the application to the child.<\/li>\n\n\n\n<li>Old passports must be submitted with new application.<\/li>\n\n\n\n<li><strong>Adopted minors<\/strong>: Submit adoption decree, foreign birth certificates must be accompanied with an English version.<\/li>\n\n\n\n<li>Photo \u2013 (2) 2&#215;2 photos taken no more than 6 months old (photo service provided at this location for 6 months and older for $18.00, <strong><em>cash only<\/em><\/strong>)<\/li>\n\n\n\n<li>A completed <a href=\"https:\/\/eforms.state.gov\/Forms\/ds11_pdf.PDF\" target=\"_blank\" rel=\"noreferrer noopener\">DS-11 application<\/a>.
